What to expect before my wellness consultation?
- Create your patient portal account and complete all of the new patient forms found in the “Questionnaire” tab. We recommend that you set aside up to 1 hour of uninterrupted time to complete and save all of the forms in one sitting, otherwise you may lose the information entered. Fill out no later than 48 hours prior to your appointment.
- Bring any existing labs to your appointment.
What to expect during my wellness consultation?
- Discussion of your medical history, symptoms, and wellness goals to understand your overall health.
- We may use kinesiology, review lab results, or recommend additional functional lab testing to uncover underlying issues.
- Receive an in depth customized healing protocol which can include dietary guidance, supplements, in-clinic therapies, or further lab testing.
- Develop a plan for progress tracking and adjustments to support lasting results and continued support.
What to expect after my wellness consultation?
- Schedule all necessary follow up appointments with our clinic and any lab testing/imaging outside of our clinic
- Follow your protocol provided to you

- Check in for your virtual visit within your Charm patient portal 15 minutes prior to your visit. Accept the video call request when prompted.

What to expect during my virtual consultation?
- Ensure you have strong internet connectivity and are in a private location for your appointment.
- If you are disconnected, please call our office immediately for assistance.
- Your provider may recommend lab tests to gain deeper insights into your health. These tests can help identify underlying issues and guide your treatment plan.
- If needed, you may be referred to your primary care provider for physical exams, screening tests, or additional diagnostic procedures.
- Before the appointment concludes, your provider will review a personalized treatment plan tailored to your specific needs. If needed, your provider will take time after your appointment has ended to create your protocol more in depth, and you will receive it within two business days.

How do I know if I need a 15, 30, or 45 minute consultation?
- Consider how recently you’ve been seen:
- If it’s been less than 6 months, a 15-minute appointment is usually sufficient.
- If it’s been 6 months to 1 year, book a 30-45 minute consultation depending on the complexity of your concerns.
- Reflect on your current needs:
- For minor updates or progress check-ins, a 15-minute visit is often enough.
- If you have new symptoms, significant changes, or multiple questions, a 30-45 minute session may be more appropriate.
- Account for time since your last visit:
- If it’s been over a year, you’ll need a minimum 45-minute consultation for a comprehensive evaluation, and you may need to complete updated intake forms to ensure all information is current.
What if I have a question in between consultations?
In Case of Emergency
If you are experiencing a medical emergency, please call 911 or go to the nearest emergency room or urgent care facility immediately.
Non-Urgent Matters
For non-urgent questions related to your recent visit or treatment plan, you can contact us during business hours by phone or email us at hello@createwellness@gmail.com. We aim to respond within two business days. If you need to reach Dr. Nikzad specifically, please call our office or send a message to one of our team members through your patient portal.
New Questions or Concerns
If you have a new question or concern between follow-up appointments, we offer brief consultations to address these. These 5-15 minute sessions can be done in person, via video chat, or over the phone and allow your provider to review your chart and give your question the time and attention it deserves. This approach ensures we can fully support your care without taking time away from scheduled appointments. Thank you for your understanding and support of our messaging policy!
I don’t live local, can I still make an appointment?
Absolutely! We offer virtual appointments, allowing you to connect with us from the comfort of your home, no matter where you are. Through virtual visits, we can provide personalized care, including functional lab testing and customized supplement recommendations, with options for home delivery or easy access. Some of our other services can also be performed virtually. Simply call our office or inquire for more details. Virtual visits are a convenient and effective way to receive the support you need for your health journey.
